Please note, applications to reopen permits may
only be submitted by the following individuals:
1. Current Homeowner – Must provide copy of
current Property Record Card (obtained at
(www.mcpafl.org) . If the owner name on Property
Card does not reflect the current property owner,
current property owner must provide either a copy
of the recorded warranty deed, or copy of warranty
deed, along with the copy of settlement/ HUD/
closing statement. If the property is owned by a
corporation, a copy of the relative sunbiz report
for the corporation (www.sunbiz.org) must be
provided.
2. Contractor – Must be licensed to perform scope
of work specified on permit.
3. Listing Agent – Must proved copy of listing
agreement.
