THEN REVOKED THEN REISSUED BECAUSE OF CONTRACTOR
DISPUTES. COPY OF RE-ISSUED PERMIT AND PLANS REVIEW TO
FILE AND COPY TO NICOLE FOR S/M FILE.
OWNER HAS SUBMITTED PERMIT APPLICATION TO REPAIR THE
DOCK (#11300033) LOCATED ON THE OTHER SIDE OF PROPERTY
BY THE OFFICE AND RESTAURANT. I DID NOT APPROVE THAT
PERMIT ON THE BASIS OF THIS OPEN CODE CASE. WHEN
COMPLIANCE IS MET ON THIS CASE, PERMIT #11300033 WILL
BE APPROVED.

RECEIVED A COPY OF BUILDING INSPECTOR SULLIVAN'S NOTES
WHEN HE PERFORMED A BL89 INSPECTION. BELOW ARE HIS
NOTES.
NEW DOCK WAS BUILT ON TOP OF OLD DOCK
WITHOUT PERMIT OR INSPECTIONS. UNABLE TO
DETERMINE IF OLD DOCK WILL PROPERLY
SUPPORT NEW DOCK, OR THE METHOD OF
SECURING NEW DOCK TO OLD DUE TO THE FACT
NEW DOCK IS COMPLETELY DECKED. I DID
NOTE AT LEAST ONE SUPPORT PILE RESTING
ON GRADE. I ALSO NOTED DOCK ON NORTH
SIDE OF PROPERTY HAS BEEN COMPLETED
WITHOUT PERMIT OR INSPECTIONS. THERE ARE
ALSO SEVERAL QUESTIONS TO BE ANSWERED
REGARDING ADA COMPLIANCE, STEP HEIGHTS,
AND ELECTRICAL INSTALLATIONS. AT THIS
TIME, I DO NOT FEEL IT PRUDENT TO PASS
AN INSPECTION ON A PUBLIC DOCK WITH THIS
MANY UNANSWERED QUESTIONS. THE PROPERTY
OWNER WAS TOLD TO HAVE AN ENGINEER,
OTHER THAN THE DESIGN PROFESSIONAL,
PROVIDE A SIGNED AND SEALED LETTER
ATTESTING TO THE SAFE AND PROPER MEANS
OF CONSTRUCTION, AND FULL COMPLIANCE
WITH ALL APPLICABLE CODES. I HAVE ALSO
FOWARDED THIS INFORMATION TO BRIAN
CORCORAN, WITH CODE INFORCEMENT
